Abstract   Flowcytometric analysis of the T lymphocyte subpopulation from peripheral blood was performed to evaluate the immunasuppressive effect of surgical operation. Thirty three patients were divided into 3 groups: minor, major and extensive, according to the duration of operation time, the length of operative incision, the blood loss during operation and the degree of tissue destruction. The peripheral blood was sampled preoperatively, followed by two additional sampling on the postopertive first day and 7th day. The lymphocytes which were stained with monoclonal antibodies were analysed by two color flowcytometric methad. The results obtained were as follows: 1) In the minor operation group, TS cell was decreased and T4/T8 ratio was increased on the postoperative first day. 2) In the major and extensive operation groups, T4 cell and T4/T8 ratio were decreased on the postoperative first day. T3 and NK cells were decreased on the postaperative 7th day. T4/ T8 ratio was decreased on the postoperative 7th day, but the difference was not statistically significant. 3) The more immunosuppressive effect was apparent in the patients with malignat diseases than in the patients with benign diseases postoperatively. 4) Postoperatively, the transfused patients were more immunosuppressed than the non- transfused patients. On the basis of the results, it can be suggested that the postoperative immunosuppression was not observed in the minor operation group, but demonstrated in the major and extensive operation groups. To improve the effect of surgical operation, the immunochemosurgery en- hancing the suppressed postoperative immunity will be necessary, especially in the maligant diseases. On the other hand, careful surgical technique to reduce the blood loss during the operation was also thought to be necessary to prevent the postaperative immunosuppression.
